Due to the unflattened nature of the 6MV beam, traditional “2D” treatment techniques cannot be employed on most normal sized targets. The inverse planning optimizer or the irregular suface comensator has to be employed to deliver a homogenous dose distribution to the target. If using the Photon Optimizer a target volume is required. In this case the brain contour was used as the target in the optimizer and both lateral beams were slightly rotated (3deg) anteriorly to create a non-divergent scenario with respect to the patient’s eyes. Simple optimization objectives were created as follows:
PTV: 100% 30.7Gy p140; 0% 31.7Gy p150;
Body: 0% 32Gy p200
Lenses: 0% 4.5Gy p110
NTO: auto p75
Simple whole brain plan. No issues. This is a clinically acceptable plan.
