This five (5) partial VMAT arc plan utilized manually selected gantry start-stop positions which avoided entrance through the contralateral lung in the transverse view. There were few challenges with this case and the metrics with opportunity for significant improvement were the mean dose to the esophagus and improved target homogeneity. Even accounting for those metrics, this VMAT plan scored higher than every case submitted to the lung plan study that year. This high score is probably attributable to the two-stage, stacked and staggered MLC design with reduced interleaf leakage compared to previous designs and also attributable to Eclipse improvements since the study was held (namely: intermediate dose calculation). With this partial arc VMAT arrangement, there seemed to be no penalty to employing the VMAT technique on this case as it scored between the eight (8) and ten (10) field IMRT plans and the treatment time was slightly faster.
